Staff who signed in with valid AdvancedMD credentials but do not have a console role yet. Assign a role to admit them, or dismiss to drop the request (they reappear if they try again).

Changes apply on the next request. Bool cells are on/off; Note audit and Coverage use gate modes.
Cannot turn off governance or flag-management for admin.
Create extra roles here; they get a matrix column and can be assigned under Users & roles.
